| Title | The Research of the Oculomotor Nerve Anatomy in Frontotemporal Orbital Zygomatic Approach |
| Abstract | Objective:To get detailed anatomical gradation of temporal area soft tissue and date of oculomotor nerve and its surrounding structures. To explore the application and significance of oculomotor nerve. And to get data of individual anatomical relations of nerves, vessels, arach- noid and duramater in the areas of oculomotor nerve by statisties and ana- lysis. To provide the anatomical evidence and reference for operations in the areas of oculomotor nerve.Methods:1、Ten formalin-fixed adult cadaveric heads specimens after perfusion of the arteries and veins with colored silicone were dissected layer by layer under microscope via orbital-zygomatic approach. Measured the anato- mical structures in the areas of oculomotor nerve accurately and take photos. Observe and measure the base of the skull revealed the situation via the oculomotor-related anatomy of gap.2、All date were statistically processed and presented in the mean (range and measured value) styleResults:1、Temporal branch of facial nerve divided into the anterior branch, middle branch and posterior branch. the anterior branch and middle branch across the edge of the zygomatic arch,about 1cm before the Superficial temporal artery and 2.4cm( 1.8~3.2cm)before the antilobium; The facial nerve main walk through in the first and second floor in the fat pad.2、The cisternal segment of the oculomotor nerve and Oculomotor upper cavernous sinus were adhered by arachnoid,which include liequist memb- rane and oculomotor nerve vaginae. To the entrance of oculomotor nerve, the distance of anterior clinoid process and posterior clinoid process is 11.24±4.32(6.13~18.06)mm and 5.43±1.41 (2.63~7.08)mm。3、The length ,width and height of Cavernous segment of the oculo- motor were 16.52±3.08(12.86~20.09)mm,2.04±0.68(1.38~3.82) mm and 1.53±0.46(1.06~1.94)mm;To join and form Dolenc triangle, Hakuba triangle and Parkinsontriangle4、The Orbital segment of the oculomotor nerve via Central District superior orbital fissure, Before entering the fissura orbitalis superior the 2~3mm divide into rami superior and remi inferior, the distance of the oculomotor nerve and the superior orbital fissure of paries superior, medialis, lateralis were0.46士0.25(0.12~1.16)mm,1.78土0.55(0.48~3.10)mm and 12.24士2.03(9.62~15.64)mm.5、internal carotid artery- tentorium cerebelli triangle is between ectocon- dyle of internal carotid artery and temporal lobe basal part,three side length were 8.23±2.87(5.76~11.03)mm,7.24±1.87(5.32~9.62)mm,and 5.78±2.24(3.16~8.02)mm.Conelusions:1、Oculomotor nerve flank gap have very big application value in dealling with Basilar artery and branches aneurysm.2、In cavernous part, Parkinson triangle and Hakuba triangle approach can discover all the structur.But the oculomotor nerve damage rate is high. |
